• We do not store any medical records or clinical documentation on our systems.

• All clinical notes and information are entered directly into your cardiologist’s electronic patient database.

• Medical data and observations are shared exclusively with your nominated cardiologist and are never disclosed to third parties.

All monitoring notes and observations are stored within the manufacturer’s patient profile, under comments or notes, and are not retained by Heart Care Connect outside of that secure system.

Heart Care Connect provides data analysis and interpretation to support your care team, however:

• We do not make clinical recommendations or decisions

• We do not offer advice to your cardiologist regarding patient care

• We cannot be held responsible for clinical decisions made based on the data we provide

All clinical decisions are the sole responsibility of your treating medical team.
