Unlike large monitoring services where thousands of patients are managed at once, we keep our caseload smaller so we can give your heart the attention it deserves. That means faster recognition of important changes, quicker help with connection issues, and real answers when you need them.
We also believe your care should go beyond the device. Our online community and regular catch-ups give you a space to connect with others, share experiences, and feel supported.

With over 10 years of experience caring for patients with pacemakers, defibrillators, and loop recorders, I’ve supported thousands of people through their heart health journey. I’ve worked alongside leading cardiologists and device specialists, building expertise in every stage of care — from implantation and follow-up, to remote monitoring.
I started Heart Care Connect because I believe patients deserve more than just technology — they deserve a service that is proactive, personal, and focused on peace of mind. My role is to make sure your device is monitored carefully and that your doctor gets clear, accurate information when it matters most.
In my spare time, you’ll often find me in my happy place — riding the waves of the ocean. As the founder of Peninsula Surf Sisters, a women’s surf community, I’ve seen firsthand the power of connection in transforming people and communities. That same passion for building supportive connections is what I bring to Heart Care Connect.
Emma Graham

My background as a paramedic means I understand what patients go through in moments of urgency, fear, and uncertainty. I’ve also worked in health communication, helping people navigate complex health topics with clarity and confidence.
At Heart Care Connect, my focus is making sure every patient feels supported and cared for, not just monitored. Whether it’s answering your questions, guiding you through device connection, or creating spaces for patients to connect with each other, I’m here to make the experience easier and more reassuring for you and your family.
In my spare time I enjoy hiking, exploring the coast, and spending time with family. I thrive on being around people and hearing their stories and I bring that same curiosity and care to supporting every Heart Care Connect member.
